Skilled Nursing

All nursing practices performed by Loving Hands Homecare Pediatric are part of a comprehensive plan of care that is set by your child’s physician. We believe that managed care does not end with medical professionals. A core part of care includes working with the parent or caregiver – educating, interacting with, and learning from the person who knows the patient best.
We work with hospital case managers to facilitate the discharge process
We ensure all equipment, and doctor’s orders have been coordinated
We work closely with DME (Durable Medical Equipment) companies to coordinate any necessary medical equipment and supplies
We are proactive in keeping physicians, hospital case managers, and insurance case managers informed of changes in a child’s condition or status
We understand the importance of maintaining children at home allowing them to thrive and remain out of the hospital
We are experienced at collaborating with schools to ensure attendance by medically complex children through the aid of an accompanying nurse

Our simple 5 step approach

What to Expect

You, or the hospital case manager will contact Loving Hands Homecare Pediatric to let us know about your needs.

Each family will meet with the Director of Operations to address their child’s clinical and social needs. This meeting can take place over the telephone, or in the hospital setting.

Our Director of Nursing, or Director of Operations will visit your home for an initial assessment and fill out any required paperwork.

Our dedicated team works with your family and child’s medical team to coordinate discharge to home (if hospitalized).

An individualized care plan is created, and nursing care begins.

We select and hire skilled, dedicated and compassionate nurses to meet individual patients’ needs.
